*If you have an eye injury, sudden onset of flashes or floaters or sudden loss of vision, please CALL the office so that we may see you in a timely manner.
*If you have any of the following medication conditions, please CALL our office to schedule your appointment so that we can provide you with ample time for you visit that your condition requires *Diabetes *Glaucoma *Macular Degeneration
*If you are 65 or older, please CALL our office to schedule your appointment.
*Missed appointments or cancellations with less than a 24-hour notice may result in a $30.00 No-Show fee.
*We do ask that when you schedule online that you provide us with BOTH your medical and vision insurance. Some eye conditions such as those mentioned above are not considered routine and therefore are not filed under routine vision insurance.
