New Patient - Eye Exam
Description:
A full evaluation of vision and eye health for individuals new to our clinic, including a review of medical and vision history. If you're interested in a contact lens prescription or fitting, these services can be performed during the visit. Additional fees apply for contact lens services.
Established Patient - Eye Exam
Description:
A complete evaluation of vision and eye health for returning patients, with a review of medical and vision history. Contact lens prescription updates or fittings can be performed if needed. Additional fees apply for contact lens services.
Pediatric Patient - Under 3 yrs of age
Description:
A gentle, age-appropriate eye exam designed for children under 3. This visit focuses on assessing visual development, eye alignment, and overall eye health. No reading or letter recognition is required. A parent or legal guardian must be present for the exam.

A comprehensive eye exam for children 6 years and older, focused on vision, eye health, and visual development. This exam involves a review of medical and vision history. Pre-Diabetic / Diabetic Eye Exam
Description:
Medical eye exam for patients with diabetes or prediabetes to check for disease-related changes.
Dry Eye Consultation
Description:
A comprehensive medical evaluation to identify the root cause of your dry eye symptoms, followed by a customized treatment plan tailored to your specific needs.

Please be aware that vision insurance and medical insurance serve different purposes.

Vision insurance typically covers routine and preventive services, such as annual eye exams and prescriptions and/or materials for glasses or contact lenses. Medical insurance, on the other hand, is used when there is a known or suspected medical diagnosis—such as dry eyes, cataracts, glaucoma, pre-diabetes/diabetes or other eye conditions—that require evaluation, treatment, or ongoing care.

While our team will do their best to explain your benefits, the final determination of coverage is made by your insurance provider, not our office. It is the patient's responsibility to to understand their insurance coverage. 

Please note: Contractually we are obligated to bill all diagnoses as found. We will not alter or adjust  a diagnosis to influence billing, as doing so would constitute fraud, which we strictly prohibit. 

I understand that a $49.00 fee for the required Advanced Retinal Health Evaluation and Macular Pigment test will be added to my comprehensive exam. I acknowledge this fee is not billable to insurance and agree to pay at the time of service.

I understand that a $39.00 cancellation fee will apply for appointments canceled with less than 24 hours’ notice. For Monday appointments, I agree to notify the office of any changes by Friday at 2:00 PM.

Important Notice

Beginning March 1, 2025, an Advanced Retinal Health Evaluation (digital image of the retina) and Macular Pigment Optical Density test (a risk assessment for Age-Related Macular Degeneration) are required as a part of all comprehensive eye exams.

These tests are an important part of our standard of care and are essential for a complete evaluation of your eye health. As such, they cannot be deferred.

A $49.00 fee will be added to each exam for these services.

Please Note: These tests will not be submitted to insurance and payment will be collected at the time of your visit.
