To schedule your Rejuvenate Med Spa consultation, a $50.00 deposit is required to reserve your appointment. This amount will be credited towards any services you choose to purchase. Please note that your consultation appointment cannot be reserved until payment has been received. We do ask that you provide a 48 hours' notice if you need to cancel or reschedule your consultation appointment in order to avoid a $50.00 missed appointment fee. At Rejuvenate Med Spa, our services are not eligible for billing through medical or vision insurance. However, you can utilize your Health Savings Account (HSA) or Flexible Spending Account (FSA) for payment for most services. If you would like to learn more about available financing options, please feel free to inquire. To submit your payment and secure your appointment, please use the payment link provided. The payment link will be available on the last page after you have selected the date and time for your consultation appointment.

This scheduler is used to schedule comprehensive eye exams only. If your insurance is not listed, please call our office for an appointment. If you need to schedule for a child age 13 or younger, please call our office. If you are scheduling for three or more persons of the same household for the same day, please call our office. If you have an ocular related disease, eye injury, illness, sudden onset flashes or floaters or sudden loss of vision, please call our office to schedule an appointment at (253) 342-1738. If you have a true ocular emergency, please dial 911 or go to the nearest emergency room. Our office requires all patients to have retinal imaging. Retinal imaging is required and is a $45 out of pocket expense due at the time of your appointment. Your visit is also subject to additional copays or coinsurances that are determined by your insurance company. Please be prepared to pay at the time of your visit. Falsifacation of information to secure an appointment through this online scheduler may result in cancellation of your appointment. Failure to give AT LEAST 24 hours notice for cancellation will result in a $50 missed appointment fee. Transaction Fee Update: Starting April 1, 2026, a 3% fee applies to credit card transactions. This fee does not apply to debit cards, HSA, FSA, Affirm, Care Credit, or cash payments.
