Visual Skills Evaluation / Developmental Eye Examination
Description:
$375 - An in-depth assessment of eye tracking, eye teaming, and eye focusing skills to determine the “visual fitness†of your/your child’s eyes. Eye Tracking Skills: evaluates adequate fixation, saccadic, and pursuit eye movement control necessary for reading, eye-hand coordination, and visual reaction time. Eye Teaming Skills: evaluates if the two eyes aim, move, and work as a coordinated team necessary for depth perception and clear single vision. Eye Focusing Skills: evaluates if the eyes can maintain clear vision at varying distances necessary for learning, reading, and sports. This evaluation will help determine whether there is a problem in any of the above areas that may be interfering with school, sports, work, or one's life. However, to determine the extent of these problems, a Visual Information Processing Evaluation may be recommended.

A glasses prescription and contact lens prescription will be given once they are finalized. Pricing varies depending on if the patient is a new or existing contact lens wearer, and type of contact lens needed. Dr. Bailey considered out-of-network with all vision insurance plans. Dr. Bailey will provide you a detailed superbill for which you can submit on your own to your vision insurance. If you have vision care insurance and would like to use your benefits towards a routine eye exam, Dr. Bailey can recommend a trusted provider close to your home. If you were referred to our office for a Visual Skills Evaluation by a primary care optometrist, to maintain professional ethics protocols, Dr. Bailey is ethically bound to return you and your family to the referring doctor for all vision care except the special care for which you were referred. Visual Information Processing Evaluation
Description:
$750 - An assessment of one’s ability to analyze, retrieve, interpret, and act upon visual information. A VIP Evaluation is necessary to determine the extent in which your/your child’s visual perception may be affecting performance. The VIP Evaluation is used to assess the whole person and determine the best course of treatment. Exact tests given during the VIP Evaluation are based on prior testing and the individual’s needs. The assessment may evaluate the following: Gross Motor Awareness Spatial Awareness Laterality Directionality Visual Perception Visual Motor Integration Visual Perception Speed Auditory Perception Reading and Spelling Dr. Bailey believes in a multidisciplinary approach to health care which may include a referral to an occupational therapist, physical therapist, audiologist, speech and language pathologist, neuropsychologist, psychiatrist, physiatrist, chiropractor, osteopathic physician, or other medical professional. The VIP Evaluation allows us to collaborate or refer for other services that may help the patient develop, enhance, or regain visual skills as quickly as possible. A Visual Skills Evaluation performed by a developmental optometrist is required prior to a Visual Information Processing Evaluation. You will need to submit a copy of the assessment for review (within the last 12 months) prior to your visit. Complete VIP assessment includes: 90-minute VIP evaluation, 3 hours grading and written report by the doctor, and 1 hour in-person or video parent/patient consultation to discuss results and next steps.

$210 - A vision therapy program consists of weekly 1:1 in-office sessions (about an hour) and the number of sessions needed to improve visual function is determined by the Visual Skills Evaluation and Visual Information Processing evaluation. Dr. Bailey creates a personalized in-office protocol in addition to home therapy activities that help to reinforce what is learned in the vision therapy room. Patients must complete a Visual Skills Evaluation prior to scheduling vision therapy sessions.

Myopia Consultation
Description:
$225 - Please choose this option if you/your child had an eye examination within the last 12 months and can provide a copy of the most recent eye examination record (including the glasses prescription that was prescribed and anterior/posterior ocular health results) prior to the visit. This consultation includes an analysis of myopia, eye focusing skills, eye tracking, and eye teaming skills. Dr. Bailey will then discuss the most appropriate interventions based upon the individual’s needs. If a myopia management program is pursued, the ‘Myopia Management Consultation’ fee will be deducted from the chosen treatment plan.

InfantSEE is a public health program managed by the American Optometric Association (AOA) designed to ensure that eye and vision care becomes an integral part of infant wellness care to improve a child's quality of life.

Annual Visual Skills Evaluation (Post-VT or Post-initial VSE)

Once a vision therapy program is completed, it is important to ensure that the skills learned in the vision therapy room are maintained over time. Dr. Bailey will discuss important visual habits needed to maintain healthy vision. A prescription check is included in this visit. If you/your child completed an initial visual skills evaluation but did not complete a vision therapy program, please contact our patient care coordinator to determine which evaluation is most appropriate for you. If you were referred to our office for a Visual Skills Evaluation by a primary care optometrist, to maintain professional ethics protocols, Dr. Bailey is ethically bound to return you and your family to the referring doctor for all vision care except the special care for which you were referred. Dr. Bailey is considered your "optometric vision therapy only doctor." Records must be provided and reviewed by Dr. Bailey. If records are not received by the appointment time, the examination will be updated to a full Visual Skills Evaluation. It is the patient's responsibility to ensure records have been requested and submitted to our office on time.
