Failure to provide valid insurance information may result in the cancellation of your appointment. If you have an eye injury, sudden onset of flashes or floaters or sudden loss of vision, please call the office to schedule an appointment. We do require a 24 hour notice for rescheduling and cancellations. No-show or last minute cancellations will result in a $50 missed appointment fee.
