IMPORTANT INSTRUCTIONS- PLEASE READ BEFORE SCHEDULING
1.) Please be certain to enter your email address correctly on the next page as the system will send confirmation(s) to the email address that you enter. You will be sent instructions for your appointment via the email address you enter. If you do not receive the email please check your spam folder.
2.) ALL NEW PATIENTS: You will receive an email immediately after scheduling your appointment. The subject of the email will read "Your appointment has been successfully booked." This email will contain a link to your electronic paperwork. This information must be filled out online within 24 hours of scheduling your appointment. If the electronic paperwork is not filled out within this 24-hour period, the system will automatically remove your appointment and make it available to other patients without exception.
3.) One exam/service time per patient.
4.) Maximum two (2) family members scheduled on one day- no exceptions.
5.) We ask all scheduled appointments that you do not bring visitors to the office with you at this time. The only exceptions to this request will be for someone who provides essential physical assistance for the patient or a parent/guardian who is escorting a patient who is a minor. All other visitors will be asked to remain outside the office.
6.) Only those with a scheduled appointment for any service will be allowed entry to the office at this time. Those without an appointment will not be permitted entry.
7.) If you intend to use vision benefits and have not received your benefit information or otherwise do not have your benefit information such as ID numbers, etc. please do not schedule an appointment until your benefit information is active and readily available. All vision benefit information is processed prior to your visit.
8.) Masks must be worn at this time. If you cannot/will not wear a mask for any reason, please schedule your appointment for another time when masks will not be required.
9.) Dr. Pelosi sees patients starting from age five.
10.) Please be considerate to other patients and be prompt to your appointment. Our schedule does not allow for late-arriving patients.
11.) We have been and remain very diligent in keeping our office safe for us and all of our visitors. We ask that if you or someone you reside with are not feeling well, you or someone you reside with has been exposed to someone who has tested positive for COVID-19, or you or someone you reside with has tested positive for COVID-19 that you please contact us to reschedule your appointment. No missed appointment fee will be charged in these circumstances. We realize schedule availability is as far out as it is but we will make very effort to reschedule your appointment as soon as possible in these circumstances.
