Notice

Prior to scheduling your appointment, please be advised:

  • For all new patients we are requiring a reservation fee of $42.00 that can be applied towards your visit. The fee is non-refundable if the appointment is missed. Please contact our office to pay, or you can go online to our website. https://visionsourcerio.com. If the reservation fee is not paid within 24 business hours the appointment will be canceled.
  • It is standard of care in our office to perform an Optomap test. It is retinal imaging that takes pictures of the back of the eyes which helps our doctors detect any life-threatening diseases like cancer, stroke, and cardiovascular disease and facilitates early protection from vision impairment or blindness. Fortunately, most vision plans cover this testing with only a $39.00 copay.
  • Unless under the age of 18 or in need of physical assistance, ALL patients must come ALONE to their appointment. Additional family members/friends will not be permitted in the office.
  • A maximum of TWO members of one family may be scheduled on the same day.
