Vision Therapy Evaluation - New Patient (Don't see an appointment? Call our office to schedule!)
Description:
You must have a documented annual eye exam with an optometrist or ophthalmologist (pediatrician screenings and school screenings do not count!) within one year of appointment before scheduling a vision therapy evaluation. Have your eye care provider send us exam notes, or schedule with us!
