Annual Exam - New Patient
Description:
If selecting this exam type, please select a Vision Insurance Plan when filling out your patient details. Some medical insurances may have routine eye exam coverage (such as Medicaid), but typically most medical insurance plans can only be used if you have a medical condition (dry eye, macular degeneration, cataracts, glaucoma, etc.).
Annual Exam - Returning Patient
Description:
If selecting this exam type, please select a Vision Insurance Plan when filling out your patient details. Emergency Appointment (ADDITIONAL FEES APPLY FOR NON-EMERGENCIES. Call our office for details.)
Description:
Examples of eye emergency: eye injury, severe irritation, or severe pain. If you do not see a same-day appointment available, please call our office and we will fit you in!
Vision Therapy Evaluation - New Patient (Don't see an appointment? Call our office to schedule!)
Description:
You must have a documented annual eye exam with an optometrist or ophthalmologist (pediatrician screenings and school screenings do not count!) within one year of appointment before scheduling a vision therapy evaluation. Have your eye care provider send us exam notes, or schedule with us!

If you have an eye injury, sudden onset of flashes or floaters or sudden loss of vision, please call the office if you do not see an appointment available within 24 hours. This form uploads securely to our patient care database.
